How to make it

  2. Click the link icon, optionally check "Shorten URL," and copy the link.
  3. Paste the link into the field above.
  4. Set a size that fits your poster or handout, then download the PNG.
  5. Print it on a classroom poster, event sign-in sheet, or slide, and test it with your own phone first.

Tips

Use the shortened forms.gle link
The "Shorten URL" checkbox in the Send dialog produces a shorter forms.gle address, which makes for a slightly simpler QR code pattern than the full docs.google.com link.
Turn on "Collect email addresses" if you need respondent contact
This setting lives in the form’s Settings tab, not in the QR code itself. Turn it on before you generate and print the code, since you cannot add it retroactively to responses already submitted.
Size for the viewing distance
A classroom poster viewed from a few feet away needs a larger code than a table tent someone picks up and holds close. Print a test copy at the target size and scan it from the expected distance before finalizing.
Limit to one response for sign-up sheets
In the form’s Settings tab, "Limit to 1 response" stops the same person submitting twice. It requires respondents to sign in to a Google account, so leave it off for public feedback forms where that would put people off.

That is fine. The link stays the same when you edit questions in an existing form, so the printed code keeps working. It only breaks if you delete the form or change its sharing link.

This tool only accepts links that start with docs.google.com/forms/ or forms.gle/. If you pasted a different kind of link (like a Google Sheets results link), go back to your form, click Send, and copy the form link instead.

Not with this input, but the general URL QR code generator accepts any form link, including Microsoft Forms and Typeform.
